Reducing the signs of ageing with PDO Monothreads

These days, many less invasive non-surgical facial rejuvenation practices have been developed to attain a more youthful appearance. These include lasers, dermal fillers, neuromodulation, fat grafts and absorbable threads.

Many people often has a misunderstanding when considering THREADS as an alternative to improving skin laxity. Most patients think that thread procedures are like a facelift or that it can hold back their ageing features.

The traditional approach to facial rejuvenation has always been facelift surgery. However, this can involve a lengthy recovery period with side effects such as scaring. Restoring lost volume and correcting facial structures can also be achieved by dermal fillers. However, to address the quality of the skin, more specific treatments can be used.

In the early 2000s, as a replacement to surgery, threads were introduced with midface rejuvenation using such threads as Gore-Tex and polypropylene. These were permanent threads which did not absorb. These permanent cog threads or lifting threads reported many complications to include asymmetry, visible, palpable threads, pain, thread migration, granuloma and skin irregularities.

It was not until early 2011; Korea introduced the PDO (polydioxanone) threads for facial rejuvenation. Already used in surgical suturing, PDO thread is a biocompatible and bioabsorbable material. Biocompatible means that it is not harmful or toxic to living tissue. PDO disintegrates over six months with the advantage of having low tissue reaction.

When inserted under the skin, PDO threads dissolve and cause-specific changes to the surrounding tissue. These changes are a fibrous merging effect of neo-collagenesis, fat reduction, tissue contracture and an increase in blood circulation. All of which have been shown to create tightness and rebuilding of elastin. PDO threads re-activates the skins normal regenerative process including mechanical stimulation of wound healing and tissue contraction.

Stimulation of elastin is the result of fibroblast activation and has the following benefits of tightening pores, brightening skin, and increasing the volume of the skin. Elastin fibres help the skin return to its normal position after being stretched.

There are two types of threads used at Face Plus Medispa. The outcome is different depending on the type of PDO thread used.

Mono PDO threads are smooth threads and provide a tightening and rejuvenation of the skin. These smooth threads do not provide any substantial lift to the skin. They are placed superficially under the skin, often in a mesh-like fashion and are suitable for patients who want to improve the firmness and quality of their skin. Treatment areas include the upper and lower cheeks, forehead, jawline, neck, nasolabial folds, marionette lines, arms, thighs and abdomen. The Mono PDO threads are performed by our team of experienced and highly trained Registered nurses.

Cog threads have hooks and are lifting threads. The hooks clasp the skin creating a lift and suspension of sagging skin. These threads promote the mechanical effect (lifting) and the biological effect (neo-collagenesis) in one treatment. Our surgeon, Dr William Mooney, perform cog thread lifts. Thread lifts are suitable for patients who have good soft tissue volume and desire a revitalisation effect. Suitable areas include jawline skin laxity and facial slimming. These threads are not suitable for patients with excessively saggy skin.

Studies have shown that absorbable threads are an excellent method in restoring youthful vigours. In particular, when combined with other methods of rejuvenation such as dermal fillers, anti-wrinkle and PRP.

Threads are low risk as they are non-invasive with virtually no scarring.

The Thread Process:
  • Within four weeks – new collagen fibres have loosely connected and merged with pre-existing fibrous connective tissue. There is an increase in blood circulation
  • After 12 weeks – the collagen fibres become denser and form a three-dimensional scaffold
  • After 24 weeks – PDO threads have biodegraded. Remodelling of new collagen is ongoing for up to 12 months.
The Thread procedure:
  • Before treatment – your skin is cleansed thoroughly, and local anaesthetic cream is applied for approximately 20 mins
  • During treatment – PDO threads are inserted using a needle under the skin. The needles are removed, leaving the thread behind.
  • After treatment – your skin is cleansed again using an antiseptic; you may use ice to soothe the treatment area, mild tenderness. Sleep slightly head up.
  • Side effects – temporary bruising, swelling, erythema, slight pulling, or prickling sensation, complications are rare.
  • What to avoid for the first 24 hours after – any sources of heat including sun exposure, no massages, no oil-based moisturisers or make-up, no exercises, no sweat
  • What to avoid for the first five days – no heat, no facials, or massages, keep the face free from sweat
  • What to avoid for the first three weeks – saunas or heat treatments
  • No anti-inflammatories for seven days before treatment
  • No local infections around the procedure
  • Implants in the procedure area
  • A propensity to form keloid scars
  • Pregnancy; lactation
  • Previous non-biological materials in the treatment area
  • To extend the effects of threads:
    • Drink lots of water (at least 2L daily) – collagen requires adequate hydration
    • Stop smoking
    • Wear sunscreen
    • Yearly treatments may be performed if required

Some of the tips that we share with you in this blog will be relevant to most ages but what we are trying to say is that it’s really important to start these tips and habits sooner rather than later!

1. You’ve read, seen and heard this a million times BUT drink lots of water! Not just any water though, read more HERE Part one and Part two HERE.

2. Be bold! Your 20’s is a great time to experiment with different makeup and hair looks. But try not to wear too much foundation/coverage. If you’re covering something up, now is the perfect time to be correcting any skin concerns you may have. That bag? It can wait!

3. Do not over pluck your brows. Keep them under control. HERE are out four steps to perfect brows

4. Quit smoking!!! It’s a bad habit that greatly impacts the health of your skin. It restricts the supply of blood to the skin and not to mentioned “smokers” lips! As you age smoker’s lips will become more prominent.

5. Take lukewarm water showers over hot showers. The heat from hot showers dry both your hair and skin. As we age the natural oils on the scalp will decrease, try washing your hair less frequently to preserve the oils. Two words, dry shampoo.

6. Don’t neglect your neck and hands. We see a lot of patients who took very good care of the skin on their face but completely neglected their neck and hands. You’ve been warned.

7. This one may just be a beauty myth but we don’t think it is. Drinking from a straw gives you wrinkles! A lot of people drink from a straw due to teethe whitening BUT any kind of repetitive movement does cause wrinkles.

Turning twenty is such a defining life moment. You are no longer the teenager, and with that comes a level of relief – but also a moment of nervousness. Should being twenty-something FEEL any different? Should I have my life figured out?

The deeper you delve into your twenties, the more this question comes up – 25 (oh God closer to 30 than 19), 26 (everyone I know is getting engaged and I haven’t even finished Uni yet), 27 (why do hangovers hurt so much all of a sudden?), 28 (since when did exercising become such a chore?), 29 (okay, one more year for me to get my life in order before I hit the big 3-0).

Sound familiar?

Your twenties are an amazing age – they are known to be some of the most life-defining you will experience. Time to study your real passions, define a career path, travel the world, move out of home (unless you’re living in Sydney, in which case it’s move BACK home) and fall in love.

BUT your twenties can also become a trap for trickiness when we start thinking they are the time we are meant to have our lives all figured out. Comparisonitis is a biggie too (thanks social media!) – because being in your twenties sees friends and loved ones making HUGE life decisions – buying a house, working long hours in a career, getting engaged and married, starting a family. For some of us, these things are amazing and something to look forward to, while for others, this is enough to want to move overseas and never look back (another common twenties trend). Either way, your twenties are a time to really find what makes YOU happy – and not living for anyone else.

A common trend I see with my twenty-something clients is this notion of comparing our lives, not only our friends and families who are seemingly more ‘successful’, but to complete strangers online. People we don’t even know. This comparisonitis goes so far that many of us twenty-somethings are even comparing how we look to the filtered, edited images we see online.

This body image comparisonitis has become an alarming trend for women in their twenties (men too) and can have really serious effects. No longer are eating disorders a common trend among teenagers only – eating disorders are very real in young people in their twenties and up. What these mental challenges stem from are feelings of not being ‘good enough’ or worthy. Looking at yourself in the mirror and not liking what you see. This isn’t made any easier when we are engaging in a digital world where it is acceptable for us to ‘swipe right or left’ on a prospective partner, judging mainly by what we see.

So, how do we counteract these feelings of low self-esteem and comparisonitis in our twenties?

Here are some quick tips for you:

  • Do a social media audit. Review your Instagram feed for a day – any post that makes you feel BAD about yourself, rather than inspired and happy – unfollow. You have the power to control social media – it doesn’t have to control you. The minute you are feeling bad about something you are reading or seeing online, hit that unfollow button. On that, only follow pages that make you feel GOOD about yourself. It’s all about Self love, read more HERE
  • Start to engage in activities that we used to in our teens – things that make us truly happy! Did you used to like Art class in high school? Then find a local class and start up again! Was writing your thing? Start journaling again! We get so caught up in the notion of ‘growing up’ that we start to leave pieces of ourselves behind – your twenties is a time to re-engage with these things and embrace your inner child!
  • Spend time with people who make you happy and feeling good about yourself. Chances are you will start drifting from high school friends in your twenties – this is normal and okay. Cherish the friendships with people who light you up, make you laugh and inspire you. Don’t spend any time with people who make you feel any less

Too often we place unrealistic pressure on ourselves to have our lives sorted by the time we are in our twenties – when in fact, life is just beginning! Take it easy and you will come to see that these years are some of the greatest of your life.

We know that a lot of men and women have been avoiding muscle relaxants such as Botox and Dysport in fear of not having movement (frozen, expressionless face) or other people being able to tell “they’ve had work done”.
Cosmetic nurses have been successfully treating fine lines, wrinkles and many other concerns related to aging, for years.

“Brotox”, how does it work? It works by blocking the signals from the nerves to the muscles. The injected muscle is unable to contract and this makes the wrinkles and lines soften and in many cases, disappear.

The treatment inclusive of a consultation will take approximately 20 to 30 minutes. If you’ve had muscles relaxants with any of our nurses in the past the treatment time will be significantly less.

Immediately following the treatment your treating cosmetic nurse will advise not to rub or touch the injected area for a total of 4 hours, they may also tell you not to lie straight down. The Botox or any muscle relaxant will generally take 7 to 14 days to kick in.

Another concern that is often brought to our attention is “How long it stay in my body, where does it go after it’s been injected?” Once the connection between the nerve and the muscle where it has been injected is blocked the “Brotox” does not remain in the body, it is cleared out. Botox stays only where injected, it does not roam through the body but attaches to the muscles where injected.

The effects of the treatment will last between 3 to 6 months, we even have patients who only require a yearly treatment.

Many of our male patients often decide to have “Brotox” due to pressures from their partner (I am speaking from experience). Following the treatment men are surprised with the amount of movement they have, yet there are no visible lines and wrinkles.

Another change that they may notice is their mood, muscles relaxants have been shown to lower feelings of depression, anxiety and irritability. The study did note that patients did not see a significant difference in their appearance, suggesting the boost was not down to a lift in their self-confidence.

When the skin is healthy it is better able to fight the signs of aging, is quicker to heal and blocks off potential diseases better than unhealthy skin.

A properly tailored skin care regime is not just about making the skin look great, it’s about overall health. Poor skin care habits will lead to rashes, sores, acne and wrinkles, making your skin for susceptible to disease and infection.

So, what do you need to stop and start?
STOP just using anything and everything! This could be causing serious long term damage to your skin.
STOP doing nothing, soap in the shower doesn’t quite cut it.

START taking proper care of your skin.
This means you need to start using:
A cleanser – as you get older you may find that you are developing more blackheads, redness and random breakouts. This is due to not cleaning your skin properly. Cleanse your skin morning and night.

Moisturise – Your skin needs moisture to survive! Try not to use hot water as heat draws the moisture out of your face, opt for warm water. Apply a moisturiser twice daily, morning and night.

The final shave before your wedding day is probably one of the most important shaves you will ever have to complete.

Before we continue, if you have a beard don’t feel the need to shave!
Probably not the best time to be trying out a new style. We do have a little advice for the bearded man, make sure you have some oil and a comb with you on the day, you will be photographed all day and you want to make sure your beard is in fine form, even if you’re not aware or ready for a photograph to be taken. On that note let’s move on to shaving…
If you generally shave and you’re not happy with how your skin appears it’s really important to take care of your skin, it will only get worse with further irritation.

As the ladies have a hair trial, have a trial facial hair trial about 3 weeks before your wedding day. Pop on the full suit and tie and trial a clean-shaven look, a little stubble or a full 5 o’clock shadow. Take a couple of snaps in daylight and see which look you prefer, looking in the mirror and taking pics are very different so make sure you take some happy snaps outside in daylight.
Take note of how many days it took for the perfect amount of stubble to grow.

It’s often a great idea to make time to have a professional shave. You will want to trial this process also so you can monitor and trial the growth of hair.

If you do opt to shave yourself and you are using a disposable razor, make sure it’s a fresh one each time you perform a trial shave and the final shave, that way you know you will achieve the same result each time. A disposable blade should not be used more than 5 times, at this point the blade will become blunt and cause the skin irritation.

If you generally use the same bar of soap front to back and top to bottom, then we can’t ask too much of you, we know BUT we strongly suggest you book in sometime for the Face Plus Medispa “Groom the Groom” package.
We have allocated two weeks for this program but you are only required to come in once each week. The first week, for your muscle relaxants and the second visit will be for your facial.

Sometimes the importance of a groom’s role in wedding planning can be overlooked. It is easy to get caught up in catering to the bride’s desires: what is her dress going to look like? Who is in the bridal party?

Brides have often dreamed of their ‘special day’ from a young age, and while this is a wonderful time to make all of their dreams come true, it is just as important to ensure the groom is looked after during this special time as well.

Remember – they have invested their own time, financial contribution and heart into planning and delivering your special proposal – and that is not to be overlooked! It takes courage, commitment and a whole heap of love to deliver this special moment – which deserves celebration in itself.

Here are some top tips to ensure the groom is just as involved and looked after in the lead up to the special day:

Allocate tasks to the groom

It can be super easy for the bride-to-be to get carried away with wedding planning – it is their dream day after all! Before getting too far ahead, sit down with your hubby-to-be and ask him what he envisioned for your special day. What colours did he imagine the bridesmaids wearing? What kind of music does he want playing? It is easy to assume your significant other doesn’t really care about these sorts of things, but you may be surprised! By asking him what he envisions, you are allowing him to participate in all of the big decisions – which you can then allocate tasks to. For example, my fiancé is really passionate about photography, so he has been given the task of choosing our photographer.  Give him tasks to focus on that he will enjoy, so he doesn’t feel left out.

Ask his advice

No, you don’t have to show him a picture of your final choice of dress, but before you go looking it may be nice to ask what styles he envisioned you in. How does he like your hair? What flowers does he like? Remember that look of love you will see at the end of the aisle – you want him to be blown away by how you look, so hearing what he has in mind is always helpful.

Remember the day is for BOTH of you

Does he like cigars or have a penchant for whiskey? While these may not be YOUR thing, remember the day is for you both. If arranging a little special bar of goodies him and his mates will enjoy will make his day that extra bit special, so be it. Attention is most likely going to be on you for the whole day anyway, so it is important he feel just as special.

Organise some special surprises for him in the lead up to the day

Perhaps you want to thank him for just being an amazing fiancé, or show him how excited you are to be marrying him. If you have followed the timeline shared in our “Bridal Ready: What and when” you will know that in the four to three weeks leading up to the big day we highly recommend you take advantage of our “Isle Ready” package (if you have not already started one of our 6 or 3 month plans).

The “Isle Ready” package includes eight LED Light treatments and one Oxygen Glow or Collagen Facial with additional LED light add on.

The “Isle Ready” package was of course, specifically designed with the bride’s time constraints in mind. Taking just 20 to 30 minutes per treatment, it is a treatment suitable for all, including those with generally healthy skin to anyone suffering from a breakout. Not to mentioned the 20-minute power nap that can be achieved during this treatment!

Available across all Face Plus Medispa clinics the LED light therapy treatment targets skin texture, fine lines and a variety of skin conditions.
Originally developed to speed up the healing process of wounds and post-surgery scars, it was also found to be an incredible rejuvenation tool for skin, too.

The cells in the skins dermis layer convert the light energy to fuel cells that work to rejuvenate the skin. The fibroblast cells in the dermis layer of the skin produce collagen and elastin, the LED feeds these cells allowing them to commence the production of collagen and elastin, as it would have in your youth(teens and early 20’s)!

You will see incredible results following your series of treatments. The complexion will appear plump with the new collagen, targeting your fine lines and new elastin that will bring that “snap” back into your skin.

Following your series of LED light treatments in the final days leading up to your wedding you will have your final facial. In the “Isle Ready” package, we offer two varying facials, this is to target the varying concerns all brides face.
The most popular facial prior to the big day is the Oxygen Glow facial, this is probably most popular as brides have already completed a course of treatments with Face Plus Medispa and they simply just need that facial for that lovely natural glow on the day. The Plumping Collagen facial is often to those who need a little extra plump and hydration. Both beautiful facials, we highly recommend you speak to your dermal therapist so they can best execute your treatment plan in the weeks leading up to your big day.

The Edible Beauty, Puff Away tea has been designed to target puffy eyes and skin. Like all of the teas in the Edible Beauty range the Puff Away tea is also DELICIOUS!
The diuretic herbs help minimise puffiness, and revive tired eyes and skin. This tea beautiful mix also contains the Amazonian tree leaf, Guayusa, which contains twice the amount of antioxidants as green tea. Guayusa is traditionally used to detoxify the blood and aid digestion. It also lowers cholesterol and blood pressure while strengthening the lungs.

All of the ingredients are pure and wildcrafted, so that only the most natural and effective herbs are used.

More detailed information regarding the key ingredients –
Guayusa: As mentioned above there are a number of key benefits to this native tree leaf that has been brewed like tea by indigenous people in the Ecuadorian Amazon for thousands of years, it’s rich in nutrients including theobromine, a compound that is associated with assisting in reducing inflammation while also working as a diuretic.

Dandelion leaf: In traditional medicine the dandelion leaves are often used to support kidney health. Research suggests the leave contain several antioxidant compounds, such as luteolin, caffeic acid and chlorogenic acid, this may help destroy potentially harmful free radicals.

Juniper Berry: They act as a mild diuretic. The diuretic action of the berries is attributed to its essential oils, in particular, terinene-4-ol, which is often known to accelerate the filtration activity of the kidneys.

The tea also contains spearmint and nettle.
